I am having the same issue appear once I installed the latest Beta Version 4.1.0.179. I have two caption buttons appearing in the title bar of both Firefox 3.6.3 and IE 8.

Thanks, Ondrej. Everything else is working perfectly. No problems with any of my web sites, over 170 sites that I have passwords for. No problems with the favicons showing up. All is working fine with the best password manager on the web.
